Once you provide information about yourself and the deceased, Jackson will check for other contracts/policies and ensure all are included in the claim. Jackson can only start the claims process once this form is submitted.

How the claim process works

 

1. Submit claim

To start the claim process, click "Initiate a Claim" and fill out the claim request.  After submission, Jackson will review it, collect any additional information, and send the claim form packet.

2. Review claim

After the beneficiary completes and returns the claim form packet, Jackson will review it.  If required, we will make contact for additional information or documentation.

3. Settle claim

Once all claim requirements are received and in good order, the settlement will be processed.

A funeral home assignment is a financial arrangement where the proceeds from a life insurance policy or annuity contract are designated to cover the costs associated with funeral services.

Please note, when a contract/policy is owned by an entity or held in custody, all claim paperwork will be sent directly to the address we have on file for that entity or custodian.
